b.  Check the appropriate box (Add, Change or Delete).

c.  Type your Department Name, Department Number, Last Name, First Name and Middle Initial in the corresponding boxes.

d.  Type two questions and their corresponding answers in the Information Item Description/Value boxes (see examples below). Choose questions that may not be known by another co-worker.
